The month in brief
Planning is underway to re-engage the sector, continue discussions and begin co-designing a new model of care to improve the hospital and primary care interface for older people in The Prince Charles Hospital catchment area including Chermside and surrounding suburbs.
‘Do it Now’ activities continue to progress with cross-sectoral working groups forming to improve the awareness, accessibility and sharing of advance care planning documents and recognition of the trajectories of dying and end of life.
Hospital and primary care interface co-design event announced
All general practices in The Prince Charles Hospital catchment area will soon receive an invitation to participate in the co-design process for the Initiative, commencing with an event in May 2019 and continuing until late September 2019.
GPs, practice nurses, practice managers and practice owners will be invited to come together with hospital specialists, other clinicians and providers to design a new model of care to improve the interface between hospital and primary care in the region.
All co-design activities will have an education and service awareness lens and encourage participants to build new models underpinned by evidence-based practice and population health data.
