Update on hospital and primary care interface
Co-design continues on the three new models to be trialled as part of the Ageing Well Initiative. The models are:
Specialist input into RACFs
A proactive response to care for the older person ensuring holistic management throughout their health journey. GPs identify and refer patients in RACFs requiring increased Hospital Specialist input and Geriatricians respond providing outreach care for patients in their own home (RACF bed) in partnership with their GP.
A trial will start this month with Estia Health, Albany Creek.
The ‘Virtual Huddle’ is a case conference between Geriatricians, GPs and the teams caring for complex older people in the community. The ‘Virtual Huddle’ provides another avenue for GPs to seek advice for the proactive, long term management of complex patients without the patient having to be in the room or attending an outpatients appointment.
GP referral into Hospital in the Home (HITH)
GP direct referral to Hospital in the Home enables older people requiring intravenous antibiotics or Warfarin bridging with an expected treatment duration of less than 7 days to be accepted directly to HITH without waiting for treatment in ED.
Model development continues with all trials aiming to commence in the coming months. Watch this space for updates as trials commence.
